Departments must have approval of the Records Manager prior to destroying records. The form is fillable; in order to use the fillable form you must download and save the form.

This form should be filled out by the Record Coordinator, signed by the appropriate manager, routed through the Records Manager who will review and fowarded it to the University Archivist prior to delivery of boxes.
