Section VIII. Safety/Loss Control
A. Safety and Loss Policy
The humanitarian and economic issues resulting from accidents demand that accident prevention be given importance in every phase of University activities. The objective is to prevent accidents by reducing or eliminating the unsafe acts or conditions that cause accidents. The objective will be attained through training and education.
Manager includes president, vice president, administrator, dean, assistant dean, director, superintendent, department chairperson, assistant chairperson, division manager, lead worker, head instructor, coordinator, or supervisor, whether classified, exempt or faculty.
b. Immediate Supervisor
Immediate Supervisor includes managers and persons occupying first line supervisory positions. This may include faculty, laboratory assistants and/or graduate assistants when acting in supervisory roles.
Employee includes all full and part time employees, students in laboratory classes or at work experience stations. Employee also includes contractors or subcontractor employees while assigned to University projects.
a. Safety Committee
The President shall appoint a Safety Committee comprised of members of all portions of the campus community to give guidance to the University’s safety effort.
b. Safety Officer
(1) The President shall appoint a University Safety Officer.
(2) The Safety Officer shall:
(a) Coordinate the development of a training and safety promotion program for managers, supervisors, and employees.
(b) Coordinate the activities of the Campus Safety Committee.
(c) Collect, analyze and distribute data on the overall accident trends for the University and its various units.
(d) Assist managers in developing safe work practices and procedures for employees.
(e) Advise University officials regarding compliance with federal, state, county, and municipal safety laws, regulations and codes.
c. Technical Safety Officer
(1) The President shall appoint a director of the Technical Safety Office.
(2) The Director of the Technical Safety Office shall:
(a) Staff, equip and direct an office to manage the use of radioactive materials and radiation producing machines and the generation, storage, recycling and disposal of radioactive, hazardous and infectious wastes.
(b) Serve as the radiation safety officer for the University.
(c) Prepare proposed policies and procedures for the use of radioactive materials and radiation producing machines and the generation, storage, recycling and disposal of radioactive, hazardous and infectious wastes.
(d) Provide advice or training to University officials, faculty, staff and students on compliance with the laws and regulations governing the use of radioactive materials and radiation producing machines and the generation, storage, recycling and disposal of radioactive, hazardous and infectious wastes.
d. Managers will:
(1) Ensure that the necessary safety equipment and protective devices are provided for each job.
(2) Enforce established safety rules, practices and procedures, and ensure that employees receive the training, instruction, and guidance necessary to assure compliance.
(3) Distribute safety information in a manner that will reach and affect every employee or student under their supervision.
(4) Review quarterly accident summary reports and take appropriate action when accident trends are unfavorable.
(5) Hold meetings with supervisors and employees to review safety conditions, safety procedures and specific hazards.
(6) Conduct job safety analysis for positions under their supervision to develop safe work practices and procedures.
(7) Ensure that proper safety precautions are taken for safe use when new tools, equipment or procedures are introduced.
(8) Ensure all accidents are reported and that proper reports are submitted.
(9) Insist that an employee see a physician before filing a worker’s compensation claim or when a re-injury occurs which may affect the same part of the body as a previous injury.
(10) Verify that a physician’s release has been granted before an injured employee returns to work and ensure that the physician understands the specific physical requirements of the position.
(11) Review time-loss accidents to verify that accident causes have been investigated and proper corrective action is taken.
(12) Include the ability to work safely in employee performance evaluations.
(13) Seek and follow the policies, procedures, advice and training provided by the Technical Safety Office regarding the use of radioactive materials and radiation producing machines and the generation, storage, recycling aand disposal of radioactive, hazardous and infectious wastes.
e. Immediate supervisors will:
(1) Enforce safety rules, practices, and procedures and ensure that each employee understands that violations will not be tolerated.
(2) Take prompt corrective action whenever unsafe conditions or acts are noted; immediately report conditions beyond your control to the appropriate manager for corrective action.
(3) Ensure that employees understand and observe safe work practices and procedures.
(4) Instruct current and new employees on safe work procedures and emphasize specific job hazards and how to avoid them.
(5) Inspect equipment and work areas and observe employees performing daily tasks to determine unsafe work habits, conditions, and general housekeeping in the work area.
(6) Insist that employees report every accident whether or not an injury occurred.
(7) Determine the cause of each accident whether or not an injury occurred.
(8) Ensure that employees receive first aid treatment on all minor injuries and, whenever necessary, transport injured to a medical facility on any injury where there is a question as to its seriousness.
(9) Ensure that first aid kits are properly stocked and accessible at all times, and understand the fundamentals of proper use.
(10) Know the location, accessibility, and use of all fire extinguishers and fire fighting equipment in their area of responsibility.
(11) Investigate and submit accident reports within 24 hours of every accident.
(12) Discuss safety with employees as necessary to promote safe work habits, discuss specific job hazards, and request safety suggestions from employees.
(13) Evaluate employee performance for the ability to work safely.
(14) Seek and follow the policies, procedures, advice and training provided by the Technical Safety Office regarding the use of radioactive materials and radiation producing machines and the generation, storage, recycling and disposal of radioactive, hazardous and infectious wastes.
f. Employees will:
(1) Understand and follow safety rules established for their protection and the protection of their co-workers.
(2) Follow specific safe work practices and procedures given by their immediate supervisor or manager.
(3) Be alert to unsafe conditions and report them immediately to their supervisor.
(4) Use common sense in daily activities and at no time do anything which knowingly could result in an accident or injury to themselves or others.
(5) Not attempt to do their job when their physical condition or mental attitude would not permit safe performance.
(6) Use all safety equipment provided and ensure guards are in place before operating machinery.
(7) Take care of tools, equipment or vehicles placed in their charge and report any needed repair or replacement necessary for safe use.
(8) Not attempt to operate, repair, or otherwise tamper with tools, equipment or vehicles without specific authorization.
(9) Report all accidents immediately to their supervisor whether or not an injury occurred or a physician’s attention is necessary.
(10) Obtain authorization, if practical, from their supervisor before seeking medical treatment.
(11) Seek and follow the policies, procedures, advice and training provided by the Technical Safety Office regarding the use of radioactive materials and radiation producing machines and the generation, storage, recycling and disposal of radioactive, hazardous and infectious wastes.
4. Corrective Action
a. Deans, department chairpersons, and supervisors shall take corrective measures as necessary to ensure that safety rules and safe work practices and procedures are not violated. Initial corrective measures include employee training, instruction and guidance to achieve a positive attitude toward working safely.
b. In every preventable accident the employee shall be made aware that the University will not tolerate unsafe work practices. Unsafe work practices include:
(1) Failure to use available personal protective equipment.
(2) Failure to use proper lifting techniques.
(3) Failure to observe hazardous footing conditions affecting the ability to accomplish a task safely.
(4) Careless or thoughtless acts.
(5) Horseplay or practical jokes.
(6) Misuse of equipment.
(7) An employee under the influence of alcohol or drugs.
c. In situations where there is a willful disregard for safety rules or the employee fails to accept and follow safe work practices, disciplinary action may be considered. Disciplinary action may include verbal reprimand, written reprimand, disciplinary probation, suspension, or dismissal.
The following should be considered when determining whether disciplinary action is warranted:
(1) Action leading to the severity of the incident.
(2) Unsafe acts and contributing negligence.
(3) Previous accident record.
(4) Safety rule violation.
(5) Repeated or willful disregard for safety rules.
(6) Work and safety attitude.
(7) Training and experience.
(8) Environmental conditions and degree of exposure to hazardous conditions.
(9) Use of personal protective equipment.
(10) Immediate supervisor’s responsibility to have avoided the accident.
5. Department Safety Goal/Incentive Awards
It is the goal of the University to eliminate or reduce the number and severity of accidents through promotion of safe work practices within the University. The goal requires the full cooperation of management, supervisors, and employees at all levels.
Safety and training programs reduce accidents as well as the cost of operation. However, the first-line supervisor and employee have a major opportunity to recognize and eliminate unsafe conditions and work method before an accident or injury occurs. Management attitude must be positive to motivate first-line supervisors and employees to accept their responsibility for safe work practices.
The Public Safety Department will have safety plaques or certificates of merit available for departments or colleges which may wish to acknowledge their department or an employee for their safety record or safety suggestions. Depending on the impact of the suggestion in reducing the University’s accident rate, a monetary award may also be considered.
