To declare Radiographic Science as your major please download the form from the above link. Make sure you view the form with acobat reader or it will be difficult to fill out correctly. Once completed you can either save it and email it to Erin Selleneit or print it off and bring to the office. Please allow a few days for the process to complete. Erin's email address is:

Here you will find the application for admission to the Radiographic Science Program. This link will become active in early January and will be active until the due date passes, which is May 15th of the year you plan to apply. Please download the application, fill it out, and then print it off and either mail to the department or bring it with the application fee.

In addition please note that we do require an OFFICIAL ISU TRANSCRIPT and all other OFFICAL COLLEGE TRANSCRIPTS be sent to the above address. Electronic copies of transcripts may be sent from a university's Registrar office directly to No electronic copies of transcripts submitted by applicants will be accepted.

For the top 30 applicants the admission process also includes an interview. The interview is brief and is similar to a job interview. Applicants should dress professionally for the interview and come prepared to answer questions from a panel of interviewers. The Career Center at ISU is a valuable resource for preparing to interview including performing mock interviews. More information can be found at:
